How much does this cost?

Tembo accepts Medicare and Medicaid. If you are insured by traditional Medicare, there is generally no additional cost to you beyond your co-pay. Our care is covered by many major insurers, and we also have self-pay options available.

Who’s on the care team?

Every patient is assigned a care navigator trained in dementia and complex care. They serve as your guide and assistant throughout your healthcare journey. Tembo will then match you to a dedicated care team based on your needs. Your care team may include a clinical nurse practitioner or geriatric physician, a behavioral and mental health specialist, a therapist, a pharmacist, and more!

What if I already have a PCP?

That’s great! We understand the PCP is an important partner in a patient’s care. Therefore, we work with them, not against them, to support and cover gaps related to dementia and complex care. We act as an extension of the PCP office to complement their care. And if you don’t have a primary care physician, we’ll help you find one.

Do I need a diagnosis to get care from Tembo?

No, our clinicians can help you manage a broad range of neurocognitive conditions regardless of stage. Those include depression, anxiety, Alzheimer’s Diseases, and related dementias.

For Providers

How much will it cost?

As a provider, Tembo services have no upfront cost. We only deduct a flat fee from the reimbursements of actively engaged patients in our services. For providers involved in value-based care, we bear the risk with you to capture any upside in savings.
