FCA partners with Tembo Health so that patients living with dementia and their families can receive a diagnosis, plan of care, find local services, 24/7 virtual urgent care, and access up to $2,500 in caregiver support.
These services are available for free for patients on Traditional Medicare as part of CMS's GUIDE program.

Together with your Care Navigator, you can plan your goals, receive dementia education and information about local services.
With your permission, we enroll you in the GUIDE program from Medicare to be able to offer all the services for free.

Under GUIDE, you receive all of the services for free if you are on Traditional Medicare and you live at home or in an Assisted or Independent Living Situation.
No, we will help you get a diagnosis, through a comprehensive assessment and ordering the necessary imaging and lab work.

1) The person living with dementia needs to have a Dementia diagnosis, but we can help get one if needed.
2) The person living with dementia needs to have Traditional Medicare as a primary insurance (i.e., no Medicare Advantage plans or special needs plans).
3) The person living with dementia can't be on hospice, in PACE, or living in a nursing home.
If you’re on IHSS or any other medicaid program, you don’t lose those benefits. GUIDE is considered a independent program that does not affect other medicare and medicaid programs..
Every patient is assigned a personal care navigator trained in dementia and complex care.
They serve as your guide and assistant throughout your healthcare journey. Tembo will then match you to a dedicated care team based on your needs. Your care team may include a clinical nurse practitioner or geriatric physician, a behavioral and mental health specialist, a therapist, a pharmacist, and more!
That’s great! We understand the PCP is an important partner in a patient’s care. Therefore, we work with them, not against them, to support and cover gaps related to dementia and complex care.
We act as an extension of the PCP office to complement their care. And if you don’t have a primary care physician, we’ll help you find one.
