Frequently Asked Questions

How much does this cost?

Under GUIDE, you receive all of the services for free if you are on Traditional Medicare and you live at home or in an Assisted or Independent Living Situation.

Do I need a diagnosis before I start?

No, we will help you get a diagnosis, through a comprehensive assessment and ordering the necessary imaging and lab work.

Afterwards, our clinicians can help you manage a broad range of neurocognitive conditions regardless of stage. Those include depression, anxiety, Alzheimer’s Diseases, and related dementias.

Who is Tembo Health?

Tembo Health is an older adult-focused telemedicine provider dedicated to keeping older adults happy, healthy, and at home. Tembo Health offers 24/7 telemedicine support, including urgent care, dementia care services, remote monitoring, and innovative technologies like hands-free vital sign monitoring and fall detection. Tembo Health partners with healthcare organizations to provide comprehensive and accessible care to older adults.

Tembo’s unique dementia care services empower families and individuals navigating the complexities of Alzheimer's and other dementias. Tembo Health provides expert care navigation, connecting patients with
the right resources and specialists. Their technology-driven solutions help track symptoms and changes in condition, allowing for timely interventions and proactive care. In short, Tembo's platform empowers families through providing direct support, addressing care gaps, providing clear clinical services, and real-time health alerts. Plus, many of their services are free to those with traditional Medicare or Medicaid as Tembo Health’s an inaugural participant in CMS’s GUIDE program– an initiative designed to help those with dementia and their families live healthier, more fulfilling lives.

What are the eligbility criteria?

1) The person living with dementia needs to have a Dementia diagnosis, but we can help get one if needed.

2) The person living with dementia needs to have Traditional Medicare as a primary insurance (i.e., no Medicare Advantage plans or special needs plans).

3) The person living with dementia can't be on hospice, in PACE, or living in a nursing home.

If I enroll in GUIDE, would I lose any of my other services such as IHSS?

If you’re on IHSS or any other medicaid program, you don’t lose those benefits. GUIDE is considered a independent program that does not affect other medicare and medicaid programs..

Who is on the care team?

Every patient is assigned a personal care navigator trained in dementia and complex care.

They serve as your guide and assistant throughout your healthcare journey. Tembo will then match you to a dedicated care team based on your needs. Your care team may include a clinical nurse practitioner or geriatric physician, a behavioral and mental health specialist, a therapist, a pharmacist, and more!

What if I already have a PCP?

That’s great! We understand the PCP is an important partner in a patient’s care. Therefore, we work with them, not against them, to support and cover gaps related to dementia and complex care.

We act as an extension of the PCP office to complement their care. And if you don’t have a primary care physician, we’ll help you find one.

What does support look like?

Clear plan for care: We'll help you create a plan to take good care of yourself or
the person you care for.

Finding help: We can help you find people to assist with your care or, if you're a
caregiver, give you a break.

Finding resources: We can help you find available resources in your community.

Finding experts: We can help you find doctors and other people who can assist.

Brain exercises: We can suggest activities to help improve memory.

Helpful products and services: We can recommend products and services
focused on dementia and senior care.

How does the 24/7 urgent care works?

If you have a medical question, or if there is doubt whether an ER visit is necessary, you can call our licensed ER Medicine Doctors 24/7, seven days a week in any of the 50 US states.

We usually respond within 5 minutes, and call in the family members on file so that we can make a joint decision about what is best for the patient.
